A 45-minute, in-home visit for children with complex medical devices and needs, up to 12 months old.

Erica Hetrick, APRN, is committed to providing parents and infants as much time as they need, so appointments will be a one to two hour time frame of arrival, with real time text or phone call updates.
Before Your Visit
Before your visit, you will receive a pre-visit email or text link to register with our fully encrypted online medical record provider Atlas.MD to provide patient information like address, date of birth, any recommendations from hospital staff at time of discharge, and links to sign treatment consents. You will also receive a text or call the morning of the visit to confirm the time frame.
During Your Visit
During your visit, a variety of services will or may be performed:
Hygienic Standards
Arrival to your home with needed equipment that has already been cleaned. Removal of shoes and washing of hands prior to start of visit.
Transition of Care
One goal of our care is to bridge hospitalization to long-term outpatient care. This includes in-home support while gaining comfort traveling with medical equipment and entering settings where other children may have contagious illnesses. Transfer of care to your pediatric provider of choice includes both virtual transfer of records and phone/virtual consultation with that provider as needed.

Complex Medical History Review & Discussion
Comprehensive review of your infant’s personal history and medical needs, including collaboration with your specialists to assist you in forming a holistic plan of care. Primary care prescriptions are provided on request; specialist prescribed medications are not prescribed.
Medical Equipment & Supplies Review
Guidance on questions for equipment and supply providers and streamlining parent-provided medical care, as allowed by current care protocols from specialists and NICU discharge. Primary care prescriptions will be provided as needed.
Breastfeeding Assistance
Breastfeeding support is provided, including assistance with latching, pumping, and pre/post feed weights if desired.

NOTE: Payment is expected at time of service. Credit card processing (including FSA with a major credit card logo) is safe and secure within our Atlas.MD billing system. Cash payments are also accepted.
After Your Visit
After your visit, you will receive an email or text to assess for any further needs and allow for feedback on service. We include 7 days of virtual access to your provider for all your questions and concerns.
Please note we do not provide emergency services.
Parents may request an email copy of the visit summary and a superbill to be provided within one week. Superbill will be provided so families can submit to their insurance companies who may reimburse a portion of the cost based on their standard newborn well-visit reimbursement amount.
